Newly designed Plain Wire Appliance (PWA)-Mushroom Bracket Positioner (PWA-MBP) indirect bracket positioner and Microimplant Anchorage in lingual orthodontic treatment.
PWA-MBP indirect bracket positioner can be used for any type of lingual brackets (both occlusal slot and horizontal slot lingual brackets) for lingual straight wire appliance. And, microimplant provides very effective and efficient anchorage in lingual orthodontic treatment.

Recently, there has been a marked increase in the number of adults desiring invisible orthodontic appliance. In order to improve the unesthetic nature of labial appliances, lingual appliances have been developed by Dr. Fujita & Dr. Kurz etc., since 1970s. However, the technical difficulties in the lingual multi-bracket treatment process had made the orthodontists not to put their interests in it which in turn led them not to recommend it to their patients.
This has occurred because firstly, precise bracket positioning is difficult. Secondly, very precise wire bending is required due to a short inter-bracket distance. Thirdly, the treatment field is so limited that it is difficult to access, and finally, the posture of the operator during treatment is very uncomfortable. For these reasons, many orthodontists experience difficulty in performing lingual treatment. With respect to these factors, I have studied about best ways to overcome these difficulties in lingual treatment since the early 1980s. What I developed first was the IIBT (Individual Indirect Bonding Technique) in which the set up model was used for precise bracket positioning. Then I have also developed the MBP (Mushroom Bracket Positioner), which allows an easier and more precise way to determine the height, angulation and inclination of brackets at the same time on the set-up model. Also, I developed lingual plain wire appliance system. I am sure that this straight archwire concept system will reduce a lot of difficulties in lingual orthodontic treatment with the development of various ready-made lingual plain arch wires.

Anchorage control is one of the most important considerations in lingual orthodontic mechanics too. We can also use all kinds of intraoral and extraoral anchorage appliances which are used in labial orthodontic treatment. As we all know, intraoral anchorage cannot avoid anchorage loss and extraoral one needs patient’s cooperation. However, small diameter of microimplant is very useful to control the anchorage in lingual orthodontic treatment without patient's compliance.
In this presentation, I am going to show how we can approach better to lingual orthodontic treatment with plain wire appliance & microimplant anchorage nowadays. |
